Enlarged Prostate or Prostate Cancer? Understanding the Difference

BPH and prostate cancer can overlap, but urinary symptoms cannot diagnose either condition. Learn how urologists assess prostate problems and next steps.

enlarged prostate vs prostate cancer - German Medical Center

In short: Benign prostatic hyperplasia is non-cancerous enlargement of the prostate, while prostate cancer is abnormal malignant cell growth. Both can coexist, and symptoms may overlap. A weak stream, urgency or night-time urination cannot identify the cause. Assessment may include history, examination, urine testing, PSA and additional investigations chosen by a urologist.

Key takeaways

  • BPH is not prostate cancer and does not automatically turn into cancer.
  • Early prostate cancer may cause no symptoms.
  • Urinary symptoms can also come from bladder, infection, medicine or neurological problems.
  • PSA is useful but is not a stand-alone cancer diagnosis.

What BPH and prostate cancer mean

Benign prostatic hyperplasia, or BPH, is a non-cancerous increase in prostate tissue. As the prostate grows around the urethra, it may obstruct urine flow or contribute to bladder symptoms. The effect of BPH depends on more than prostate size.

Prostate cancer begins when malignant cells grow within the prostate. Many early cancers produce no symptoms. Some grow slowly, while others are more aggressive, so diagnosis must establish both the presence and biological risk of disease.

Why symptoms cannot provide the diagnosis

SymptomPossible explanations
Weak or interrupted streamBPH, urethral narrowing, bladder muscle problems or other obstruction
Urgency and frequencyBPH, overactive bladder, infection, diabetes, medicines or fluid intake
Night-time urinationBPH, sleep problems, fluid redistribution, diabetes or heart and kidney conditions
Blood in urineInfection, stones, BPH, bladder or kidney disease, or cancer
Bone pain or weight lossMany possible causes; requires medical assessment

Because symptoms overlap, a symptom checklist cannot safely separate BPH from prostate cancer. Blood in urine, inability to urinate, fever with urinary symptoms or severe pain requires prompt assessment.

How a urologist evaluates prostate symptoms

The clinician reviews symptom duration, fluid and caffeine intake, medicines, infections, family history and how symptoms affect sleep and daily life. Examination may include the abdomen, external genitalia and a digital rectal examination when appropriate.

Urinalysis can identify infection, blood, glucose or other abnormalities. PSA may contribute to the assessment, but it can rise with BPH, inflammation and other non-cancer causes. Urine-flow testing, post-void residual measurement, ultrasound, cystoscopy, MRI or biopsy may be considered according to findings.

Treatment paths are different

Mild BPH symptoms may be monitored. Behavioural changes, medicines or procedures can be considered when symptoms are troublesome or complications develop. The decision depends on symptom burden, prostate anatomy, bladder function, side effects and patient priorities.

Prostate-cancer management depends on grade, stage, PSA, imaging, age, health and preferences. Options may include active surveillance, surgery, radiation, hormone therapy or other cancer-directed care. A cancer diagnosis should not be treated as if every tumour has the same risk.

When to arrange an assessment

  • urinary symptoms persist or disrupt sleep
  • you cannot empty the bladder comfortably
  • there is blood in urine or semen
  • you have recurrent urinary infections
  • a close relative had prostate cancer
  • you want an informed discussion about PSA testing

Frequently asked questions

Does an enlarged prostate become cancer?

BPH is a benign condition and is not prostate cancer. A person can have BPH and prostate cancer at the same time, which is why new or changing symptoms and screening questions should be assessed individually.

Can prostate cancer cause the same symptoms as BPH?

Yes, but early prostate cancer often causes no symptoms. When symptoms occur, they may overlap with BPH or other urinary conditions. Testing is needed to identify the cause.

Does a large prostate mean a high cancer risk?

Prostate size alone does not diagnose cancer. BPH can increase prostate size and PSA. Cancer assessment considers PSA pattern, examination, imaging, family history and sometimes biopsy.

When is urinary retention an emergency?

Seek urgent care if you cannot pass urine, particularly with lower abdominal pain. Fever, chills or feeling severely unwell with urinary symptoms also requires prompt medical assessment.

Can lifestyle changes treat BPH?

Adjusting evening fluids, caffeine or alcohol and reviewing medicines may reduce symptoms for some men, but changes should not delay assessment of persistent symptoms or red flags.
